8a 12 month Fixed Term Contract or Secondment
Based in Crawley Hospital with travel across West Sussex area
37.5 hours per week

This is an exciting opportunity to make a real difference to the lives of vulnerable residents in Care homes and Nursing homes across Crawley and Horsham and Mid Sussex CCG localities. The Care Homes Programme aims to co-design improvements in the quality of care for these residents with partners across Health and Social Care. The post is funded by the Better Care Fund and is part of the West Sussex BCF programme leading on health and social care integration. The post is hosted by Horsham and Mid Sussex CCG and the successful applicant will work within the Strategy & Transformation Directorate in the CCG, working alongside Commissioning Managers in Social Care, lead clinicians, colleagues in Primary Care and commissioning partners from neighbouring CCGs.

The Programme Manager for Care Homes will report to the Programme Director for Long Term Conditions and lead on projects focussed on care home initiatives, delivering a programme of work to improve support for care homes and enhance their quality of care, as well as progressing local projects which align with NHS England’s New Models of Care being tested in national Vanguard sites. The expected outcomes as a result of this work are to identify potential gaps in provision; potential opportunities with our providers to more effectively support homes, and collaboratively drive forward the adoption of local and national innovations.

The role requires developing the already established programme of work to ensure a structured, evidenced based approach to care home initiatives, which results in improved consistency in delivery, demonstrable value for money, and improvements in residents’ wellbeing and experience of care. This should also decrease the burden of reactive care whilst reducing the numbers of non-elective admissions and the risks and costs associated with poor care quality.

Crucial to this role is the ability to interrogate and interpret complex data sources to inform targeted initiatives, alongside robust programme management skills. The successful candidate will be a confident leader, resilient and able to work at a fast pace whilst developing excellent collaborative relationships with commissioners, colleagues and external partners.
